The Hitomi - Astro-H - Satellite

It seems the Astro-H satellite, designed for researching black-holes and x-rays' from with a new device called an x-ray microcalorimeter, suffered a major mishap. The proverbial jury is still out on what exactly has happened to it, but there have been quite a few reports of some sort of debris being found in orbit that shouldn't be there. The latest news is from EarthSky:

The loss of the Hitomi satellite is a major, and expensive, blow to researcher's across the planet.
