2 star Alignment

I'm having a few probes with my heq5 synscan. I have done a pretty accurate polar alignment and balanced and levelled scope. First star vega is quite a bit off and I have to manually centre it using ra and dec levers. 2nd star kochab still not quite centered. Now when I slew to an object it's off centre, any ideas why or how I can improve this. I've done it before and didn't have these problem, very annoying!
