orion reflex sight alignment issues

Can any one give me an idea as to why my reflex sight will not align according to my eyepiece? Ive adjusted both alt and az and they wont move any further in both directions!?

Ive added two washers between the shoe and the reflex mount! Kinda worked but not an ideal solution for a product that should work out of the box! It seems theres too much play in the shoe/dovetail and mis'alligns when tightening it down!
